Initial Consultation

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, treatment can be done the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation prior to your treatment appointment.

Please assist us by providing the following information at the time of your consultation:

1. Your endodontic referral slip from your dentist.  You may bring copies of X-rays, however, we may take additional X-rays at various angles to aid us in our diagnostic evaluation.
2. A list of medications you are presently taking.
3. If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.
4. If you are a patient who requires premedication prior to dental care, alert the scheduling coordinators of your needs and properly premedicate prior to your consultation and treatment appointments.

IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation and treatment visits. Furthermore, they must remain on the premises throughout the entire procedure.

A pre-operative consultation and physical examination is mandatory for patients undergoing IV anesthesia for root canal therapy or apical surgery.

Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.)
